Information for Referring Providers
Health Infusion Services partners with physicians, nurse practitioners, and other licensed prescribers to deliver safe, timely infusion therapy in a patient-centered setting. We function as an extension of your office, following your treatment plan while keeping you informed every step of the way.
How to Refer a Patient
To refer a patient for infusion therapy:

Complete the Infusion Order Form
Include diagnosis, medication, dose, frequency, route, premedications, and any hold parameters.

Attach Supporting Documentation
Recent office notes, relevant labs, imaging, and prior authorization approval (if available).

Send Documentation
Fax or securely email the completed order set and supporting documents to our office

We Handle the Rest
Our team will verify benefits, contact the patient to schedule, and keep your office informed of the plan and any issues that arise.
Clinical Capabilities
Our team is experienced in administering:
- Specialty IV medications (e.g., IVIG, biologics, iron, osteoporosis agents, and other therapies appropriate for outpatient infusion).
- Premedication and monitoring per your protocol.
- Infusions requiring routine vital sign checks and post-infusion observation.
All infusions are administered by licensed nurses under the supervision of a nurse practitioner and/or collaborating physician, following your specific orders and our internal safety policies.
Communication & Documentation
We prioritize timely, clear communication with referring offices. Our process typically includes:
- Confirmation of receipt of orders and scheduling status.
- Documentation of medication, dose, administration times, vitals, and any reactions.
- Post-visit infusion summaries transmitted to your office.
- Immediate notification in the event of an infusion reaction, treatment hold, or patient concern.
